New Student Registration Procedures

A warm welcome to all students that will be registering to attend PRSMS for the 2012-2013 school year. In order to make your registration experience as smooth as possible, listed below is what is required for a successful enrollment. We look forward to meeting you!

REGISTRATION PROCEDURES

OUT OF STATE & FIRST TIME FLORIDA STUDENTS

Students entering a Pasco County Public School for the first time at any grade level MUST present the following items upon registration:

*Proof of residency (such as a utility bill, lease agreement, mortgage statement with the last 30 days PLUS a driver’s license with the current address. Cell phone, cable bills, etc are not considered proof of residency)
* Current immunization record – Hepatitis B series and TD Booster within the last 5 years (Tdap booster required for incoming 7th grade)
*Proof of a physical exam dated & signed by a physician within the last 12 months
*Copy of a birth certificate
*Copy of a social security card
*Copy of a report card or withdrawal grade form. Please include records for all grades completed (6, 7 & 8)
*Special education records (if applicable) to include copies of current IEP and psychological report
*Legal guardianship records (if applicable)

In-State Student Transfers

Immunizations

ATTENTION PARENTS OF SIXTH GRADE STUDENTS:

Please be aware that all students entering the 7th grade are required by the State of Florida to have had a Tdap immunization prior to entering school. A student who does not have the required proof will be excluded from school until documentation is provided. Immunizations can be obtained from your own doctor or through the Health Department and must be on a DH 680 blue form.

Immunizations can be obtained at the Pasco County Health Department free of charge. You can contact the local Health Department clinic at (352) 521-1450 or consult your private pediatrician for further information.

ONLINE LUNCH APPLICATION

Free/Reduced-Price Meal Application needs to be completed each school year. The application can be picked up from your school cafeteria or you can fill out the form online. (clicking on this link will open a new window)

PAY FOR LUNCH ONLINE

Please be aware that a student may only charge lunches up to $3.00. After that, an Alternate Lunch will be provided for your child. This meal consists of a Cheese Sandwich, 2 sides and choice of milk. To avoid your child receiving this meal, please make sure that they have lunch money with them or on their account.
